About this Virtual Instructor Led Training (VILT)
Accidents in the oil and gas industry can result in serious consequences for personnel, operations, and reputation. A systematic and well-executed investigation not only helps identify the root causes of incidents but also contributes to organizational learning and safety improvement. To meet regulatory requirements and strengthen operational resilience, it is essential for safety professionals and operational leaders to be equipped with advanced investigation and reporting skills aligned with international standards such as OSHA, API, and HSE guidelines.
In this 5 half-day Virtual Instructor Led Training (VILT) course, participants will learn how to conduct thorough accident investigations—from immediate response, evidence preservation, and team deployment, to root cause analysis, witness interviewing, and report writing. The VILT course will also cover techniques such as fault tree analysis, 5-Why, and barrier analysis, while addressing system-level failures and human error factors. Participants will develop actionable recommendations, understand legal and documentation requirements, and learn how to communicate findings effectively to internal and external stakeholders. The VILT course will be supported by real-world case studies, scenario-based exercises, and interactive workshops.
On completion of this VILT course, participants will be able to:
- Apply systematic investigation frameworks to manage major and minor incidents effectively.
- Utilize analytical tools (e.g., FTA, Fishbone, Barrier Analysis) to determine root causes of complex events.
- Recognize and mitigate human error by understanding behavioural and organizational influences.
- Conduct professional interviews and gather credible, admissible evidence from diverse sources.
- Develop legally sound and action-oriented final reports to communicate findings and corrective actions.
- Identify gaps in risk control systems and suggest sustainable organizational improvements.
- Present investigation outcomes to internal and external stakeholders with clarity and confidence.
This VILT course is suitable for officers who may become involved in these investigation as well as managers wishing to have a deeper knowledge of this process and crucially how it affects the interpretation of the law, regulations and company procedures (HSE Officers, Supervisors, Inspectors, Advisors, Auditors, Team Leaders/Managers).

This course is delivered through live virtual instructor-led sessions that combine expert presentations, guided discussions, and structured case study analyses. Participants will explore real-world incident scenarios and apply investigation principles in a collaborative learning environment. Interactive elements are designed to reinforce key concepts and ensure practical understanding of advanced investigation and reporting standards.
Your expert VILT instructor is a highly experienced occupational safety and health (OSH) professional with over 23 years in the field, beginning his career in prevention, firefighting, and first aid before moving into health, safety, and emergency rescue—particularly within oil exploration and extractive industries. He currently works as a Senior OSH Consultant and Project Coordinator, having led key safety and emergency management initiatives across high-risk industrial sites in countries including Congo, Burkina Faso, Ivory Coast, Gabon, Mauritania, and Bulgaria.
With formal training in emergency response and crisis management, your expert VILT instructor has developed and implemented comprehensive HSSE (Health, Safety, Security, Environment) management systems tailored to the needs of major oil and mining companies. His work includes the creation of emergency response departments, operational protocols, and training programs, as well as technical support in incident investigation and compliance auditing. Major projects include SEMAFOR (2020), where he developed emergency response capabilities for a mining operation; CAROIL (Gabon, 2018), focusing on OSH system reorganization and ISO audits; and BON MARINE (Bulgaria, 2015–2016), where he oversaw the creation of an integrated OSH management system for deep-water drilling logistics.
Your expert VILT instructor holds a Master’s degree in physical security and is a certified Lead Auditor for ISO 9001:2015 (CQI IRCA). He is also a NEBOSH tutor and invigilator, an IOSH Managing Safely trainer, and a certified instructor in tools such as BowTie XP, Incident XP, and Audit XP. His training sessions are grounded in real-world operational challenges, offering participants both technical rigor and practical insight into accident investigation and OSH management tailored to the oil and gas industry.
