About this Classroom Training

Power systems have grown to become the single largest most connected system humans have ever designed. As we scale up in production, connectivity, and consumption, the impact of our failures become proportionately larger. Today a large blackout on the grid level may impact millions of users.

A failure offers a great opportunity to asset owners, engineers and operators acquire knowledge by identifying the cause(s) and use this knowledge to enhance safety and improve performance. Failure investigations often play an integral part of the total asset management process and provide essential information for risk assessments – be it related to engineering specifications, manufacturing, transportation, installation, commissioning tests, maintenance, operation, or ageing mechanisms.

This 3-day course will provide guidance for the participants on the main concepts and principals involved in carrying out a failure investigation. The course also presents interesting case study examples and is embedded with interactive sessions such as mock-up failure investigations to encourage participants to practice what is learnt throughout the course.

This course will be delivered face-to-face over 3-day sessions, comprising of 8 hours per day, 1 hour lunch and 2 breaks of 15 minutes per day. Course Duration: 24 hours in total, 24 CPD points. Maximum number of participants: 25

By the end of this 3 days training course, participants will be able to:

  • Discover the general failure investigation reporting aspects.

This training will comprise a combination of lectures (50%), case study sharing (30%), and interactive activities (20%). Classroom activities and exercises will be conducted at least twice daily over the course of three days. Participants are encouraged to engage in interactive Q&A sessions for brief inquiries. Dedicated time will be allocated after each main topic for deeper discussions and Q&A sessions. Interactive Q&A sessions are highly encouraged. The trainer will supplement the training with an example template for a failure investigation report, along with materials covering data analysis and best practices for evidence preservation/documentation. Basic investigation tools such as a multimeter, portable digital microscope, vernier caliper, and photography scale will be utilised. Additionally, Sologic® RCA software (sandbox version) will be employed for activities, and various online apps such as Cluedo, Menti, and Slido will be incorporated.

Your expert instructor is an experienced forensic investigator specialising in the Root Cause Analysis of power system equipment failures. These investigations are primarily conducted to enhance continuous engineering practices, handle claims (insurance, warranty), and pursue litigation. As a Professional Engineer, he leverages his experience and knowledge in failure investigation to contribute to high-quality electrical engineering solutions and improvement programs.

With 15 years of experience, he has conducted over 200 root cause investigations for a diverse range of medium and high-voltage equipment failures, some of which have resulted in loss of life and property damage from fire incidents. Since 2020, as a Senior Consultant at a Quality Assurance and Risk Management Organisation in The Netherlands, with his expertise in power cables and failure investigations he has conducted investigations for international clients across Europe, the Middle East, Africa, and Asia.

In addition to his core focus, he has been involved in renewable energy-related projects, including the commissioning of grid-connected large-scale solar plants (between 2017 and 2020) and conducting a technology survey for a future grid-connected offshore wind park (in 2022). Over the past 16 years, he has conducted investigations for international clients across Europe, the Middle East, Africa, and Asia. He has led and actively contributed to over 200 projects, encompassing generation, transmission, and distribution network power failure investigations for primary equipment and accessories. These investigations cover various voltage classes ranging from 0.4 up to 500 kV.

He possesses extensive knowledge of conceptual and practical power system insulation materials, failure modes, and testing methodologies related to root cause analysis of power failures. He also experiences on topics related to failure investigations for Transmission and Distribution Network operators globally. Furthermore, he is a certified trainer by the Malaysian HRDF. Over the past 3 years, he has conducted numerous failure investigations involving RE, offshore wind, and grid-connected energy storage systems.
